Not unless there was more than one baby. It is common to see both yolk sack and baby until the placenta forms but if there are not 2 separate heart beats then there is probably only one baby.
Sadly it looks as though your baby has died at some stage and you have not yet completely miscarried. At 12 weeks the baby is still very small but it is fully formed and would be visible.
Not necessarily you could be having twins and they don't have to be identical.

Yes, it is very common for one twin to hide behind the other and be missed on the scan. That's what happened to my best friend's mum, she went for a scan suspecting she was having twins, and they told her it was one baby, when she went for her 12 week, it turned out she was actually pregnant with triplets, they hid behind each other. Yes very good chance if it wasnt proven on scan or blood tests. if you just had a lot of blood, and thought you lost hte baby then theres a good chance that you were wrong. I have had many pregnancies where Ive bled a lot. and often thought I had lost it, but I let my midwife know, and she organised a scan which showed a healthy baby with a heartbeat. you need to talk to your maternity carer and have an ultrasound. goodluck!
