Old Republic can wait. Mass Effect 3 is supposed to be the top selling game of 2012. You can get Old Republic now however, and trade it back in when ME3 comes out.
I'm a huge fan of both, and I like them equally.
It is pretty fun, but if you are old enough, you should play Mass Effect.
If you have an Xbox, I'd recommend getting Star Wars: Knights of the Old Republic (I and II). They're kinda old at this point, but they're still great games.

If you played Mass Effect 1 the second game imports your old game save, so it would depend on how you played Mass Effect 1 as to whether or not Kaiden is alive. If you did not play Mass Effect 1, then the game will default to the character with the opposite gender to Shepard. So Ashley will be there with a male Shepard, Kaiden for a female one. Star Wars: Knights of the Old Republic was made before Star Wars: The Old Republic.
Knights of the Old Republic has not yet been announced, but the Old Republic comes out fall of 2011
