You should always only turn from the lane closest to the direction you wish to turn. For example you will turn only from the far left lane if you are turning to the left. If you wish to turn right then you will have to do this from the far right lane only.
You should position your car in the right hand lane or as close to the right side of the road as is reasonable. I agree, some drivers turn left before they turn right or turn right before they turn left, that's a no-no.

You should always check to see if a bicyclist is coming up the bike lane from your right. In my opinion, you should always check right before turning right whether there is a bike lane or not. I live on a corner where I see people turn right without looking right first. I am waiting for the day when someone gets totally whacked by a car passing another car coming from the right. It is a no passing zone, but not a day goes by that I don't see at least one person passing in the no passing zone. Moral of the story, look both way before going anywhere, even on a one way street, you never know what some idiot is going to do.
You don't clearly state what movement YOUR car is making. If you are both approaching the interesection from opposite directions and both want to turn left there should be no conflict unless it's an awfully small intersection. Other than that, there is no 'right-of-way" under that scenario, and you will both have to proceed with due caution to avoid a collision. On the other hand if you are turning right, and the oncoming car is turning left and both of you intend to wind up going in the same direction, the left-turning car should yield to you. The vehicle turning right always has the right-of-way.
