If the piercing is an older seasoned piercing and the hole looks like it's enlarging, this could be due to a recent jewellery change. Not all body jewellery is good jewellery. There is a lot of cheap junk on the market that claims to be quality and is in fact junk.
Enlargement of a piercing is generally a sign of an allergic reaction to the metals the barbell is made of (generally a high nickle content). Remove the jewellery and replace it with jewellery you have used before and have had no issues with. Observe the piercing over the next week and see if the enlargement has reverted to normal. If you see a change then clearly the jewellery was the issue.
