A hernia is a tear in the muscles of the abdomen that forms a hole for the gut to slip through. So the lump that you feel under the skin is a bit of bowel. If the bowel gets pinched too tightly by the muscles or if it swells and its blood supply is cut off, or if it twists itself so the blood supply is cit off, we say that the person has a 'strangulated hernia'. This is a surgical emergency because of the very real risk of that bit of bowel becoming gangrenous.
