In no case shall the extension be such that ladder deflection under a load would, by itself, cause the ladder to slip off its support.
although going under a ladder isn't bad luck, probally someone who was walking under a ladder probally got hurt or killed walking under a ladder when something fell on him from a worker on the ladder

Originally these were designed for the mining industry to protect workers from being hit by buckets of coal and to allow for them to sit and rest when climbing long distances. That would be classed as negligence. The person working at the top of the ladder has a duty of care to make sure they work safely and considerately.
