Animals in zoos are outside of their usual environments and so can experience stress and/or boredom. Zoo Keepers must constantly invent new ways of keeping their charges occupied and engaged by utilizing things known as "behavior enrichment". These can include anything ranging from a perforated tube filled with small food pellets to an old tire or even an outdoor shower.
Also, many animals in zoos have specific health concerns due to previous injury (which could be how they arrived in the zoo's care) or advanced age. The Zoo Keeper and Staff Veterinarian work together to keep the animal in good health and accepting of any medical procedures that need to be done. Getting a domestic pet cat to sit still for an exam is one thing... getting a full-grown lion to allow a human that close is an entirely different matter!
