Christopher Columbus returned from his first voyage in 1493 with three ships: the Niña, the Pinta, and the Santa María. The Santa María was the largest and served as his flagship, while the Niña and Pinta were caravels. After the Santa María ran aground and was abandoned on Hispaniola, Columbus returned to Spain with just the Niña and Pinta.

From what i could tell this accident happened when the captain ran aground on a bed of rocks. While the passengers were scrambling for their lives, the captain "accidentally" fell into a lifeboat and abandoned the site.
In June 1770, James Cook (not yet a captain) ran aground on the Great Barrier Reef near the mouth of what became the Endeavour River.
