After the Great Schism of 1054, the main body of the Church was divided between the Roman Catholic Church, in the West, and the Eastern Orthodox Church in the East.

There were always other, independent, Churches, however, and these include the Coptic Orthodox Church and the Oriental Orthodox Church, among others, some of which continue to this day.

There were many other groups that were heretical, such as the Cathars, against whom the Church launched a crusade.

Roman Catholic AnswersUnfortunately for us, there were no census takers, no modern records, no printing press, etc. back in the Middle Ages. We do know that the Church was very strong, and that for the most part there was a parish church in every village and town of any size. The people were people of faith, and the Church was were they learned, not just their faith, but news, etc. There were an awful lot of them, and the monasteries were flourishing all over Europe. They were centers of learning and education.

There was a church in at least nearly every medieval town. Perhaps somewhere there was a town with no church, but it would have been unusual.
