A Man Hole Cover is the grid/grill that goes over a sewer hole or drain in the streets.
It is "politically correct" version of a real word: man-hole cover. A man-hole is an opening in the street designed to admit a person into the infrastructure below. The usage is not sexist, since "man" properly denotes a human being (as in wo-man) and not necessarily an adult male human.

Not sure what you mean. You're not supposed to cover the embouchure hole, but if you miss an open-hole key or don't cover it well, it will sound out of key and maybe squeak.
