The oxidation of formaldehyde: H2O + HCOH --> CO2 + 4e- + 4H+
Formaldehyde, HCHO, is an example of an aldehyde (or carbonyl compounds)
There are three elements present in formaldehyde. Namely, they are carbon, hydrogen and oxygen.
No .. but it is in Cigarettes

Mothballs could create some of the odor.
Yes, multiple chemical sensitivity (MCS) is a condition that can cause an individual to experience a heightened sensitivity to certain chemicals, including formaldehyde. Symptoms of MCS can include headaches, nausea, and a heightened sense of smell. People with MCS may be particularly sensitive to formaldehyde, which is found in many products, including building materials, plastics, paint, and furniture.
A "musty" smell would describe a damp animal
Their sense of smell.

Formaldehyde is liquid at room temperature.
Means the product has no formaldehyde in it.
No, formaldehyde is an example of an aldehyde, not protein.
Formaldehyde-free Brazilian keratin treatments are NOT really formaldehyde free. They contain a formaldehyde derivative, such as ether, which is basically chemically identical to formaldehyde. Formaldehyde is considered to be a carcinogen, specifically related to respiratory cancers, leukemia, and brain cancer.
Flowers in Formaldehyde was created in 2004.
Wash with soap and water. Rub and soak your hands with squeezed lemon or lime juice for 3-5 min. Then sprinkle baking soda on your hands and scrub vigorously. Rinse and repeat :) this should get the smell out.
It could. The formaldehyde smell is usually from outgassing from formaldehyde used in the manufacture of some of the materials used to line the safe- foam padding, carpeting, etc. The smell SHOULD go away with some ventilation for a while. Do not know the date of your question (part of a pile of older questions that the computer dredged up) but it is now 2012- a safe bought 3 years ago should not be outgassing at this time. Would try leaving the door open with a small fan blowing into the safe for a few days, and see if that fixes the matter.
