Interpretation of any constitution is not a single question that cane be answered easily. It depends on the bias of the intrepreter(s), the method employed or model used; Americans have argued about the Constitution since Day One and hopefully we will continue to do so, that's how it evolves. As for "Democratic-Republic(an)", The US is a Democratic Republic.

That is probably not going to happen. There would be a crisis if the president is not able to continue his/her term; the Constitution says the VP takes over the president's duties, but the Constitution also requires that the president be a natural born citizen. Some things would have to change before this could happen, probably in the form of some amendments.
If he is found to not be fit to stay in office, the cabinet can vote to have him removed. Then the vice president will become president. If the president can state in writing that he is fit to continue in office than he may continue. The cabinet then has 48 hours to present a case against the president, to have him removed. The reason that this is in the constitution is for medical reasons. This may not be entirely right, but that is how I understand it. Another way in my opinion would be if the president were to break his oath of office. (Which our president has done several times)
