call the local ombudsman... the phone number must be posted in your hospital.
If abuse or unsafe practices have not been reported, then notifying a manager or supervisor is recommended. Then, contacting local departments in charge of the field would be the next step. Further reporting may be required and is at one's discretion when reporting.
If a social worker has reported unsafe practice but no action has been taken, they may need to escalate the issue. This can include reporting the matter to their supervisor or manager, documenting the lack of response, and potentially reaching out to a higher authority or regulatory body. It is crucial to follow the proper channels and ensure that the concern is addressed to ensure the safety and well-being of those involved.
If a social worker reports unsafe practice and nothing is done to correct it, they should follow up with their superiors or the appropriate regulatory body to escalate the issue. The social worker has a responsibility to ensure the safety and well-being of those they serve, so they may need to continue advocating for action to be taken to address the unsafe practice. If necessary, the social worker may also need to consider removing themselves from the situation if it compromises their ethical responsibilities.
The same as anyone else. A LPN is required to report it.

Practice is already a verb. For example "to practice something" is an action and therefore a verb.

A theory is an idea and hypothesis which explains some things. Practice is an exercise and application of any task or theory or in other words translation of an idea into action. On the lighter side: Theory is where you know everything but nothing works. Practice is where everything works but nobody knows why. My wife/husband combines both Theory and Practice. Nothing works and nobody knows why.
