It's drawn in response to complaints. It depends on the complaints. 18,000 WBCs would probably be indicative of inflammation or infection. Count could be incidental when drawn as part of a complete blood count following surgery, and elevated WBCs would be expected, with no particular action based on that. Count could be elevated because of infectious agent, in which case you might expect a culture, depending on severity/symptoms, possibly with antibiotics pending culture results. These are the common things for somebody relatively healthy, but really, it totally depends on the person's history and complaints.
