Here are the three I know:

Cat-scratch fever: Affects big cats, caused by scratching post, cured with aloe vera.

Pink elephant disease: Affects elephants, caused by pursuit ball, cured with cotton candy stall.

Speedy Syndrome: Affects rhinos, caused by coffee shop, cured with tall goldenrod.

I don't know how many there are, but these are all the ones I know. (Even in the right conditions, Diseases are extremely rare to come by.)

Pink Elephant Disease (Animal: Any type of Elephant, Cause: Pounce Ball, Cure: Cotton Candy Stall)

Cat Scratch Fever (Animal: Any type of Cat, Cause: Scratching Post, Cure: Aloe Vera Plant)

Speedy Syndrome (Animal: Any type of Rhino, Cause: Coffee Shop, Cure: Tall Goldenrod)

Litterbug (Animal: Don't Know. It happened to a Warrah and a Dodo for me, Cause: Overflowing Garbage Cans and Garbage everywhere, Cure: No idea...)

Those are all of the ones I can remember. Oh, and 'Tutorial Love' which appeared in an Extinct Animals Tutorial, and will not appear anywhere else. (Animal: Dodo, Cause: Bananas, Cure: Pounce Ball

Also, if the cause of the disease is feces, test a compost building.
