A patient's chief complaint is what they say when they first walk in. When you are writing your 600, or whatever form your job has, it is best to just write down everything they say. It's easier if you are in a clinic and you can type it all down. But if you are an EMT, just write bullet points of everything they say. It is best to be complete then not write something you didn't think was necessary and then have it turn out to be something crucial in the end.
