Common examples of copyright infringement are uploading material from a commercial DVD onto YouTube, ripping and duplicating CDs and DVDs, copying and redistributing photos and text, etc.
Prosecution varies from country to country. In the US, infringers can be charged up to $30,000 per infringement (so distributing an album of, say, 10 mp3s can cost up to $300,000). That being said, very few infringement cases go to court; most are settled long before that point, usually for an amount much closer to real damages.
People who have broken copyright law are called infringers.
No; they should be prosecuted for copyright infringement.

The website is legal because in its terms of service it states that no copyright material is allowed. Therefore, when people post copyright material the website is not responsible. Downloading copyrighted material without paying is illegal no matter what website it is from unless it is allowed by the artist (such as select songs on the android market or mixtapes uploaded by artists themselves). There is little to no chance anyone downloading the music will be prosecuted but those people who post it may be forced to take the material down or could be prosecuted.
I would expect that everyone using the internet has at some time or another done something that would be considered copyright infringement, whether they knew it or not. That would be about 28% of the world population, or nearly two billion people.

People cannot be protected by copyright.
The Rwanda Genocide was one of the most disturbing events in the history of Africa. As a result hundreds of people were prosecuted for taking part in it. There is currently no one still being prosecuted as a result of this event.
