It is a more inapropriate movie. Usually some profound movies have "Unrated movies" that are very inapropriate, but they also have a pg-13 version that is not quite as profound, but has the same story line.

To my knowledge, the U.S. does not have any laws about minors purchasing R, NC-17 or Unrated films. I believe the only laws are about viewing these films in theatres. However, some video stores have enacted their own policies about renting or selling R, NC-17 or Unrated movies to minors. So you would just have to find a store that doesn't have one of these policies.
There can be many reasons a movie is listed as unrated. The film makers may not wish to send their production to be rated as a choice, or the movie may still be in the process of rating the production when the movie or show is released.
