It would be most helpful to grasp just how many people with what appear to be personality disorders really have an underlying subclinical depression which may be completely relieved with an appropriate antidepressant. I can't tell you how many people have come to me with apparent personality disorders fulfilling all DSM criteria who were either completely cured with an antidepressant and no longer needed psychotherapy; or, while having their symptoms greatly relieved, now had gained sufficient ego strength that they could be better patients in psychotherapy. And let me add that many of these subclinical depressions do NOT necessarily fulfill DSM criteria for either Disthymic or Major Depressive Disorders. 2. The most obvious sign of mental disorder is disturbances of thought and perception, for example, delusions and/or hallucinations. Delusions are strongly held wrong beliefs despite any and all evidence to the contrary, no matter how logical the evidence is. For example, the belief that you are indestructible (delusions of grandeur), despite the fact that you are bleeding from a wound or cut. Hallucinations are things you see, hear, smell, or taste that aren't really there. This should be distinguished from simple illusions, in which you might hear one sound and mistake it for another, for example, thinking you heard someone call your name, or in which you might see what you mistake as movement out of the corner of your eye. Rather, an hallucination seems as real to the person experiencing it as the real thing would be. These disturbances in thought and perception usually leads to eccentricities of behavior or speech. However, some people with personality disorders, as opposed to psychotic disorders, may seem perfectly ordinary most of the time, and many with personality disorders can lead a fairly normal life and even have careers.
