There are two basic types of optical fiber.

Multi Mode Optical Fiber

Used to transmit many signals per fiber. (Multi Mode fibers are generally used for computer networks, lan applications).

MultiMode Optical Fiber is produced as 50/125 and 62.5/125. (Core/Cladding diameter in microns.)

Single Mode Optical Fiber

Used to transmit one signal per fiber (Single Mode fibers are generally used for telephone and cable tv applications.)

Single Mode Optical Fiber is produced as 8/125 and 9/125. (Core/Cladding diameter in microns.)

1) Bending loss (Due to Macrobends & Microbends)

2) Scattering loss (Rayleigh scattering & Mie scattering)

3) Dispersion loss

4) Absorption loss

5) Insertion loss

fading consist of attenuation, dispersion, scattering, bending loss.

rest all above there is one more type named nonlinear effect.
