After four months of secret debate and many compromises, the proposed Constitution was submitted to the states for approval. Although the vote was close in some states, the Constitution was eventually ratified and the new Federal government came into existence in 1789.

The last four states to ratify the Constitution were Virginia (June 25, 1788), New York (July 26, 1788), North Carolina (November 21, 1789), and Rhode Island (May 29, 1790). Rhode Island had refused to send delegates to the Constitutional Convention in 1787. As the Constitution only need the approval of nine states to take effect, it was already the supreme law of the land at the time of the last four states' ratification.

38, or 3/4 of the states must approve of the amendment before it can me added to the Constitution. This process does not have to happen simultaneously. It's typically for the process to take several months or even years. As soon as the criteria is reached, the amendment passes. Four amendments, some as far back as 1789 are still technically pending approval.

There have been many amendments proposed for the Constitution. However to date only 27 of them have been ratified. A Wikipedia article estimates the total to be around 11,372 for the period 1789 through 2008. Of those that were never ratified, six received the required two-thirds majority votes of both houses of Congress.
US presidents are elected to four-terms. They are allowed to serve a second term if they are re-elected but there is a two-term limit due to the 22nd amendment to the US Constitution which was ratified in 1951. Before this amendment there was a precedent set by Washington for the 2-term limit, but FDR was willing to break the precedent and ran for and was elected to four terms. He died in office before he could try for a fifth term. The average per President of time served as President for all of the Presidents from Washington through Obama is 5.30 years.
