= rickettsial = and it refers to a very large category of microbial organisms that cause a variety of conditions, some of which can cause death soon enough that the question becomes moot. <g> just kidding!

From my limited experience it seems that the doctor will give you antibiotics if s/he thinks you may have a rickettsial infection based on your symptoms. If it doesn't respond then it's probably a virus. Specifically testing for the exact microbe is expensive and takes a long time to get the results back, and since they all respond to the same antibiotics, seems to basically be unnecessary except for severe and long-term cases.
