It depends on many things: what you were taking Pristiq for in the first place, what dosage, what you've tried before, what side effects you suffered from with Pristiq, etc. The best alternative (as in, most equivalant switch) would be Cymbalta which is the only other approved SNRI (subtype of antidepressant that includes Effexor, Pristiq and Cymbalta). Effexor also, but Pristiq is the "upgraded" version of Effexor already (as Lexapro is the "upgraded" version of Celexa). Only a doctor can determine which is the best alternative for you.

the answer to you questions is that people get the same side effects that they do on lower doses. I am a psychiatrist and has used cymbalta at 90mg and 120mg in very select patients. I only increase it over 60mg if the patient is having no side effects. If they start to have side effects at these higher doses, i have them lower the dose back to where they were. Generally doctors avoid SSRI and TCA combinations. Safer approaches are tried first, as switching to different agents, increasing dosages, SNRIs (Effexor and Cymbalta), combinations of SSRI and Wellbutrin, Remeron or buspirone (Buspar) or augmentation of the SSRI with modafinil (US: Provigil). Well, if these didn't help you, you can try SSRI + TCA but expect some side effects that may be irritating. Most common from this combination should be dry mouth, constipation, weight gain, and of course sexual side effects. If you have especially high anxiety, your doctor most certainly will prefer the use of SSRI + TCA from some of the above solutions.

Also, the higher the dose, the more pronounced the side effects are. I am prescribed the highest dose (120mg daily) but I typically only take 60-90mg.
