Before entering a mandir you should have had a bath/shower. At the mandir there is a specific room in which you keep your shoes on shelves.

Depends on the particular temple. Most of the time there are no rules other than that you must remove your shoes before entering. Some do not allow any leather article in the temple, some would want you to enter in traditional dresses, some would want you to bathe before you enter the temple, some would not allow a non-hindu to enter the temple. So check at the gate.

Before you plan to go to temple, you must take a head bath, wear washed clothes, go to the temple, leave your shoes, walk a little, every temple provides water to wash feet, after washing prostate at the long pillar i.e.dhwaja sthambha, then starting from left side slowly with folded hands, go around the main temple and dhwaja sthambam 3 times, 5, 11, 21,27, 108 times, according to you convenience, now stand near the main sanctorium a little side, offer your prayers, retreat come out sit for sometime, in the temple area. You are refreshed, and blessed.
