Idioms are for emphasis and interest. Using an idiom instead of just a phrase is more colorful. It sounds more interesting that way. It also adds more emphasis to what you say.

Example:

I felt very anxious waiting for the doctor. --- OR ---

I was on pins and needles waiting for the doctor
