If a person is severely ill it is seen as they are impure or "poisoned" this does not mean that they will not be cared for but generally it is taboo to enter a temple or shrine if you are injured or sick since you are bringing impurity into their sacred place. Severe illness will be typically treated by a local physician and Buddhist priest who will cleanse the sufferer of the impure spiritual affliction
