Ice storms require a combination of high humidity, changing air pressure, and far below freezing temperatures at high altitudes. Changing pressure creates rapid wind movement which can move humidity and precipitation through freezing climates to be turned to ice and the pulled down to earth by gravity.

Basically, you need to have cold air entrenched (assisted by strong high pressure to the northwest pumping in more cold air from one direction) in an area as a storm system moves in. That low pressure system will bring with it warmer air along with precipitation, particularly along the warm front. That warm air will advance higher up in the atmosphere, because warm air is less dense than cold air and can't dislodge the cold air at the surface. This "overrunning" also helps create precipitation because it creates atmospheric lift.

So what you get is snow that forms in clouds, falls through a layer of warmer air and melts, and then freezes again in subfreezing surface air. That creates ice (specifically, freezing rain).

In The Online World It Says That, "Temperatures in the clouds would be above freezing, so rain would fall. Temperatures on the ground would be just below freezing, so the rain would freeze as soon as it reached surfaces on Earth."
