'Bring up the engine speed slowly and if the vibration increases slightly until it disapears altogether the two of the cylinders are not igniting and the ignition coil pack must be changed. If no vibration can be felt then it is more likely to be a small leak in one of the vacuum lines to solve this you must listen closely under the hood for a 'hissing sound' usually on or around the intake manifold but not limited to these locations .If a vacuum line leak is detected hold a thumb over the line end and listen for a change in engine note

There can be several things that will cause your 2003 Chevy Malibu to run rough and not shift into fourth here. The probable cause is a lack of fuel. Due to a bad fuel injector.

I was told it was due to a leaky manifold gasket, which caused vacuum leak and the engine to run lean. I have not had a chance to replace the gasket yet to see if that resolves it.
