What you are describing is typically caused by a warped rotor. Sometimes a disk brake rotor will warp after it has become overheated, typically from the brakes working too hard, such as going down a long hill, that's one reason it's a good idea to use a lower gear and let the engine do some of the braking for you.

Sometimes it can be caused by loading the vehicle with too much weight then the brakes have to work a little extra hard.

Whatever the cause, warped disk brake rotors must be replaced or "turned", but many of the newer rotors are already too "thin" to turn so the only option is to replace them.
