It usually happens when you've either hit the nail or trapped it somehow - it's a bruise under the nail.
A fungi or bacterial infection in the bed of the nail can cause the nail to turn orange. Smoking cigarettes frequently can also cause the nails to turn orange.
It depends how badly you have damaged the nail bed. Sometimes the nail will grow until the dark spot (or blue spot) grow right out and other times you may lose the nail completely. Nature takes it course so wait and see, but keep the nail clean and put antibiotic cream on it.

The nail bed is part of the nail matrix called the sterile matrix. It extends from the edge of the germinal matrix, or lunula, to the hyponychium. The nail bed contains the blood vessels, nerves, and melanocytes, or melanin-producing cells. As the nail is produced by the root, it streams down along the nail bed, which adds material to the undersurface of the nail making it thicker. It is important for normal nail growth that the nail bed be smooth. If it is not, the nail may split or develop grooves that can be cosmetically unappealing.
The white area in fingernails and hair are caused by air. That's why snow is white and not blue. Little bumps and bruises that are done to the bed of the nail cause these imperfections.
