Horses are powerful animals. They can kick incredibly hard and fast, and being hit with their hooves is like being struck with a hammer at high speeds. Their teeth, though made for consuming vegetation, can also produce a rather nasty bite. Never just see a horse as a simple beast of burden. They are intelligent and not to be underestimated.
Horses spook from anything unnatural. Sunshine in an arena, a rustling feed bag, winter blankets, coats, foreign objects, strange sounds, and even another horse spooking! When I ride my horse, she spooks at the harrow, shies at the barrels (for barrel racing, of course!) or even sunshine in front of a jump. This can sometime get annoying if you are trying to jump a course and your horse freaks out at each jump! My little pony I rescued is afraid of lead ropes lying on the ground and loud claps of thunder. Every horse has a different fear.

Yes, and it occurs a lot more common than one might think. Harm can come in many forms: deliberate physical/verbal abuse, neglect of basic necessities, actions of good intentions that come only from the human's satisfaction not the horse's, communication misunderstanding between human and horse, physical correction of a bad behaviour (such as hitting a horse in the ribs to make it "smarten up"), poor decision-making based on a horse's state of mind or health which could make the situation for not just the horse but the owner as well worse than what it was; the list goes on. One can only hope that a human that owns a horse knows enough that the harm that does come to their horse or horses can only be from mistakes and not deliberation.
