It probably has fin rot. Ask an employee at the petstore, get the fish some medicine.

It's either fin loss or fin rot. Fin loss is due to their fins being snagged on rough surfaces (ie PLASTIC plants, jagged rocks). Any objects that are rough on your fingers should be removed from the bowl/tank. Fin rot is an infection that eats away at the fins of the fish. It starts as black or red tips on the ends of fins and, if not treated, can destroy the whole fin and start working on the body of the fish. Fin rot can be treated with antibiotics found at your local pet store. It should say "Treats fin rot" on the box. It's not uncommon for the fish to act sluggish and refuse to eat when the have fin rot. The drugs should also bring back their appetite.

Fin rot is rarely fatal if prevented before the case gets too serious, but in this case, since the fins are already disintegrating, I do not think your betta will live. You can try to treat it, however, by putting in a few crystals of aquarium salt and immediately transferring your betta to a different tank if it is with other fish. Also, check to see if some areas of the betta's fins are white-if they are, then this is most definitely fin rot. The best treatment I would recommend is keeping the water clean and putting a few crystals of aquarium salt in it, along with buying some fin rot treatment products at the pet store-there's one called Melafix or something along the lines of that-it's great for treating fin rot. If the fish is in with other fish then there will probably be some fin nipping going on. If the fish is by itself then the problem will be in the water parameters. I'd test the water and then do more water changes as required.
