Massive haemopericardium is caused by bleeding into the pericardial sack. The cause of this may be penetration by a sharp object or projectile, blunt chest trauma, or splitting of the heart wall as a result of mitral valve problems or damage to the myocardium (the inner layer of the heart) following a heart attack for example.

The most common cause of aortic dissection is long standing hypertension. Over time, shear stress results in a microperforation in the intimal layer of the aorta, allowing blood to dissect into the medial layer, resulting in an aortic dissection. The dissection can go proximally or distally. If the tear is close enough to the aortic root and the aorta dissects proximally, it can result in hemopericardium and myocardial infarction (from dissection of the coronary ostia).

Other causes of aortic dissection include certain diseases, such as syphilis, Marfans disease and other connective tissue disorders.
