Usually it means the flapper in the tank isn't seating properly. They do wear over time. But try cleaning it first with some soapy water. That should "buy" you extra time. If not, off to the "box store" with ye for a fair replacement. Or visit your local Plumbing supply store for a better one. (Note: I don't work at either - I'm just a humble home-repairer who has fixed more toilets than she cares to discuss.)
