You would best refer to a map in a book. Try to free-hand draw it first. If you have trouble with free-hand, make a Xerox copy of the book picture and try tracing the outline. Next, locate the city you wish to put on the map. You can use a darkened circle to designate the city. Label the circle, right above, with the city name. Label the state name. You should probably include the capitol city too--use a star to designate it and label it.
That depends entirely on how the labeled lines are spaced on your particular map or globe. There is no standard. On my mapping software, the next label north of 75° N is 75° 00' 02" N. The next label south of 75° N is 74° 59' 58" N.
