prostate cancer. usually as a rule if pain is waking you from sleep with no exceptional cause it is known as a "red-flag" in healthcare
also if you commonly have sweats during sleep and this pain linked it is fairly likely to be a space occupying lesion - tumor.
unless he hasn't defecated in a while..
