Coronary heart disease is known by many names, CHD, CAD, coronary artery disease. It is used to describe a general hardening and narrowing of the vessels of the heart due to fat and other materials. This condition results in poor circulation, and cardiac distress. Narrowing of the blood vessels can elevate the blood pressure which results in damage throughout the body, it also prevents the adequate flow to the tissues of the heart and causes damage directly to it. A healthy heart diet will help to prevent this issue.

Cholesterol is the substance associated with animal fat that causes atherosclerosis(the narrowing, or hardening, of arteries due to plaque that is formed from cholesterol build up in coronary arteries).
Heart disease typically does not cause fatty deposits in the arteries. Rather the process is the other way around: an unhealthy diet high in fat and cholesterol causes a build up of fatty deposits, and these deposits lead to heart diseases such as Coronary Heart Disease.

Obviously coronary heart disease! A blocked coronary artery reduces blood flow & thus Oxygen to the heart which in turn causes Angina when a person will experience pain & breatlessness. Without treatment a blockage can cause a heart attack.
