It can be many things.
If it is in the urine, it can be bladder infection, cancer, bladder stones, kidney stones, prostatitis, prostate cancer and even an enlarged prostate(the gland has many superficial blood vessels on it's surface). If from the anal area, a simple hemmorhoid ( usually bright red streaks on the toilet paper), colon polps or cancer, ulcerations in the colon (these produce a brighter red color). If it is from higher up in the digestive system, the blood would be darker, almost like coffee grounds. It can even come from bleeding stomach ulcers.
Be aware that some medications and foods can give the appearance of blood, like bismuth ( in pepto bismol) in the stool, iron tablets can turn stools dark, beets, foods with red dye, and over the counter urinary analgesics like Pyridium ( Azo tablets) etc.
Anytime you see blood, don't hesitate and please see your doctor asap!

A similar question posted recently received the answer that the bleeding is actually caused by a period (or menstruation) which may be the case, although you haven't said whether you are male of female. It may also be that you have a disease that has weakened your skin, which may or may not be a sexually transmitted disease. If this is not just menstruation, then it should be looked at by a doctor.
