Heart transplant is only done if the heart has been terribly damaged by infection or disease, and if there are no other ways to improve heart function.
One way a doctor treats heart disease is to do a heart transplant. That is, removing the heart of the patient, and replacing it with a healthy heart of someone who has died.
8/17/2007 Heart Disease in need of transplant.
The vast majority of patients with heart disease can be treated medically (exercise, dietary changes, medicatons) and sometimes surgically (coronary artery stents, bypasses). However, when the heart completely fails, as a pump, sometimes the only option left is a heart transplant (in order to avoid impending death).
Congenital heart disease means you are born with this disease so coronary occlusion cannot be a cause of congenital heart disease.
It is possible if Lyme carditis damaged your heart enough. But it is extremely rare. So, it is possible but unlikely.
The treatment will depend upon the type of heart disease. It will involve drugs, intervention like angioplasty and stenting, laser ablation for arrhythmia, bypass surgery and heart transplant

No physical Education will (hopefully) prevent heart disease.
Heart disease
Frequent sore throat will not necessarily cause heart disease. One episode of strep throat leading to rheumatic fever can cause heart disease.
