Their contribution to society is the same as anyone who is paid for their skills. They pay taxes which the country uses to maintain Law and Order, good roads, electricity, infrastructure, clean safe water, vaccinations, hospitals, welfare and that sort of thing. Then, as individual people, they have families, live in communities/neighbourhoods and have normal human relationships just like anyone else. When they shop locally, wealth is put back into the local community. They may belong to religious bodies such as churches or temples, as well as charitable clubs such as the Rotary Club or private clubs such as Golf clubs. They often contribute to further education by writing papers about new surgical techniques they have developed and found successful, which is a big help to other neurosurgeons, as they may use those techniques to save more lives. Basically, their contribution to society is the same as any other Professionally qualified and practising person. The only difference is that their income comes from doing brain surgery.
