These occasionally appear on the top of questions if there is some kind of a problem with the content which requires further comment to make it more balanced, or to reduce its inflammatory nature.

Such flags are put on by supervisors who have knowledge relating to the relevant question which would lead them to believe such a flag is necessary.

Only supervisors cam remove flags. Questions or answers can be flagged for a variety of reasons - usually because something needs improving - or the question/answer could be offensive. for this reason - 'ordinary' users don't have the authority to remove flags from questions or answers.
