It depends on the bird and it depends on the injury. First, as much as you want to help, approach carefully. Some birds may be ill-- and they might carry parasites or bacteria that are harmful to humans. Other birds might be scared, and not realizing you just want to help, they could peck at you. Additionally, some baby birds may seem injured, but in reality, they are fine-- wait a while and observe how the bird acts: if it can walk or hop, chances are it wants to find its mother; there's an equally good chance the mother is going to come back for her baby.
If a bird seems seriously injured and you fear for its safety (like, you don't want a cat or a hawk to eat the bird), approach it quietly and carefully. You may want to put it gently into a small box that has ventilation so the bird can breathe, and then seek medical help by contacting a trained wildlife rehabilitator. Unless you have expertise in animal rescue, your best bet is to call your local Audubon Society or Animal Rescue League and ask for instructions about what to do. They may be able to come and give the animal the medical treatment it needs to recover.
I found an injured owl once and had a hard time finding an agency to help it. Try the local zoo to see if they will take it in or a natural wildlife preserve. To catch an owl throw a towel over their head so they can't fly.
