If she is not hospitalized then spend every moment you can with her and try getting her to laugh; live life to the fullest; love each other until the end comes for her. Fill what life she has with good memories of the both of you and surround yourselves with family and friends. If she cannot be released from hospital then visit her as much as you can. Just talk to her as if she was well and keep letting her know you love her and will never forget her. Whatever your belief system perhaps tell her you'll see each other again. As for you you, the pain will be great letting her go, but be thankful you have fallen in love with her; have wonderful memories of her and once over your grief then live for the both of you.
