You have to go right, and climb the "Caterpillar Statue" again, going onto the swinging ledge, and back to the "tooth" place, which is the entrance to where you got the bone. Then you walk onto the ledge that shifts from side to side, getting to the other side of the "Tooth Ditch." Go left. You'll find a place with a drawing on the wall. At the end of the hall, you'll find a "Key Ingredient." Now go up.
Go right to the Ancient Ruins, then go left again to Booga Bay. You'll find a weird dude selling grass skirts. Click on him and retrieve a grass skirt.
Go back to the Ancient Ruins. Push the big stone block to the bottom of a bamboo tree, and try climbing up the rope and jumping on the branches without being knocked off by coconuts. Again, go up.
Now keep going up, using ropes and ledges to keep you ascending. At the top, you'll see a weird masked dude. Put on your grass skirt and click on him. He'll take away your coconut milk, your bone, and the Key Ingredient, replacing it with a coconut Calming Potion. Go down, and back to Booga Bay.
Hop over to the next island on Booga Bay and click on the steering wheel to feed the shark. Click anywhere to launch the Calming Potion. The shark will come up and eat it. Now go back.
Swim across the sea where the shark used to protect, and you'll find an island. Go right. There will be an old man nearby. Click on him, then go back left, and swim across the sea again, then back to the original Booga Bay. The lady will be there to greet you, and she'll thank you with the island medallion!
