Well, Ni Hao means hello, so, after someone says hello, you should say "Ni hao ma" In other words, "How are you?"

你好! ni(3) hao(3) also means "How are you!" but in statement form, not as a question which is 你好吗? "How are you?"

Your answer would be along the lines of 我很好! wo(3) hen(2) hao(3), lit. 'I am fine!'.
