First let me say Pressure and Volume are inversely proportional (P=1/V). When you inhale, the muscles of your chest wall, neck, and diaphragm contract, expanding your chest cavity. This increases the volume while decrease the pressure within your lung space. The pressure is actually less than the pressure outside of your body (atmospheric pressure) and so air flows in to your lungs (from high to low pressure, like all liquids and gases). Air continues to enter your lungs until the pressure in the lung space matches (approximately) the atmospheric pressure. Then, as you exhale, Your abs and some chest wall muscles contract, pushing diaphragm up and chest down and in, decreasing volume thus increasing pleural pressure. As the pressure rises above atmospheric pressure, air again flows down it's pressure gradient out of your body until lung pressure and air pressure are equivalent. Without this negative pressure mechanism, you would suffocate and die quickly.
It becomes more difficult to breathe at high elevations because atmospheric pressure is lower, meaning it is harder to create a negative pressure gradient in your lungs.
