They refer to the time - in hours. The slanted edge of the gnomon casts a shadow on the sundial at the position which represents the time.
Technically, 12 AM doesn't exist. We refer to 12 noon or 12 midnight. From 6 AM, it is six hours till noon, and eighteen hours till midnight.

Q 6 hours means that you take the medication around the clock every 6 hours, even during sleeping hours. An example would be 0900, 1500, 2100, 0300, 0900. QID means 4 times a day, but the doses are divided during waking hours. An example might be 0900, 1300, 1700, 2100.
90 degrees and 270 degrees.
It's 7 hours 4pm = 1600 9 am = 0900 16 - 9 = 7

Let's try a number line using military time. Nine o'clock AM is 0900, and one o'clock AM is 0100. So, we start at 0900 and count to 0100: 0900-1000-1100-1200-1300-1400-1500-1600-1700-1800-1900-2000-2100-2200-2300-0000-0100 We get 16 hours. Another way you can do this is simple math. We know that 0100 is an hour after midnight, and 0000 is midnight. So, take 2400 (another way to look at midnight), and subtract 0900: Simplified it is 24 - 9 = 15 + 1 = 16 9AM to 1AM is sixteen hours.
8 pm EST (There are two Eastern Standard Times in the world. I'm assuming you want the one that's on the same continent with MST.)
The military normally use the 24 hour clock, so 9:30am would be 0900 hours.
