It means how many miles per gallon you can get driving around a city type area, example, a city would have a lot more traffic lights and stop signs than a highway which takes more gas. So when you see something say a cars "City MPG" its telling you how many miles you can get with one gallon in a area where traffic doesn't keep flowing.
