Odds are, that's his automatic reaction to learning there is a female on the phone. He probably didn't think about the fact that he wasn't sitting with one of his buddies. Guys can be pretty clueless sometimes... :)

You use a question mark because it's a question. You are asking them how they are doing.
I would answer, so: "I'm doing great" followed by the thanks: "Thanks for asking" and then return the question: "How are you doing?" "I'm doing great, thanks for asking! How are you doing?"

Okay. That's not exactly a question. I'm guessing the question is "WHY does he keep bringing up my friend?" We don't know why. We can't even make a good guess at why, because we don't know the context. If he's asking about your friend in suggestive ways or constantly talking about how good-looking she is, that's a bit different than asking how she's doing if he knows you've just seen her. If it bothers you ... and it pretty clearly does ... ask him about it.
Obviously if you're asking this question, you're just doing it to be stupid.
Possibly just curious to know if you have already made plans to do something. Why not ask them what they are doing?
