∙ 14y agoThe mostly likely and common cause of head shaking in horses is flies, or gnats usually around the eyes. If you treat the face with fly wipes or a face mask and the behavior continues the problem could be ear mites. If treatment for mites doesn't fix it, your horse most likely has an ear infection or fungus, or something as serious as EPM or other neurological disorders, a vet would need to diagnosis

Horses have the ability to rotate their ears 180 degrees so he can listen in front, side, and behind him, and move his ears independently of each other. They flick when listening to things in their immediate environment. They also flick their ears if flies are biting. They can get ear mites and other biting insects. The ears need to be cleaned a couple times a week as part of the regular grooming routine. Use a soft cotton cloth and gently wipe the ears on the inside being careful not to poke down into the inner ear. After cleaning, petroleum jelly can be applied to moisturize the insides of the ears.
