If it is a vigorous shake the it is trying to dislodge something such as a fly or dust.

If it is a quivering type and the weather is cold then it is shivering and needs to have shelter and a blanket to warm up.

The mostly likely and common cause of head shaking in horses is flies, or gnats usually around the eyes. If you treat the face with fly wipes or a face mask and the behavior continues the problem could be ear mites. If treatment for mites doesn't fix it, your horse most likely has an ear infection or fungus, or something as serious as EPM or other neurological disorders, a vet would need to diagnosis

The ears need to be cleaned a couple times a week as part of the regular grooming routine. After cleaning, petroleum jelly can be applied to moisturize the insides of the ears.
